The Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) has received $1,495,100 from the Ontario government to attach up to 3,500 community members to a family doctor or primary care team by April 2026. The funding is part of Ontario’s $2.1-billion Primary Care Action Plan, which aims to connect 300,000 people across the province to primary care this year.
FLA OHT will work with Maple Family Health Team and other providers to connect residents to People-Centred Health Homes in their communities. Priority populations include pregnant people, infants, cancer patients, and those with chronic diseases. Residents without a provider are encouraged to register with Health Care Connect.
The funding also supports an interprofessional community hub to provide mental health services, physiotherapy, diabetes support, and care navigation. The hub will reduce wait times, increase primary care capacity, and help more residents access the right care close to home.
